Our 2022 back to school orientation is being held virtually.  All information is prepared and organized below into 3 steps for you to review/complete. As always, thank you for allowing us the opportunity to be your partners in your child(ren)'s education.  We are ready to Explore! Excite! and Empower!  2022-2023 here we come!

Complete Back to School Forms & Payments

Pay online

  • Elementary: Select your child's  homeroom teacher.

  • Middle School: Select "Middle School"

Complete an emergency contact form for each child that attends Mayfair Lab.

Students MUST be registered for extended day BEFORE attending.
