- 2 proofs of residency dated within the last 30 days from the district approved list (water bill, electric bill, gas bill, residential telephone bill, signed lease agreement, monthly mortgage statement)

- report cards showing grades for the most recent 4 semesters (usually the final report cards from the most recent 2 school years)
