ALL Applicants Must Submit:

- 2 proofs of residency dated within the last 30 days from the district approved list (water bill, electric bill, gas bill, residential telephone bill, signed lease agreement, home insurance bill, monthly mortgage statement)

4th - 8th Grade Applicants Must Also Submit:

- report cards showing grades for the most recent 4 semesters (usually the final report cards from the most recent 2 school years)

- Math and ELA score reports for the most recent norm-referenced standardized assessment (example: LEAP2025 or TerraNova)
