Administration's Orientation Information

Pay online

  • Elementary: Select your child's  homeroom teacher.

  • Middle School: Select "Middle School"

Complete a NEW emergency contact form for each child that attends Mayfair Lab.

Complete Back to School Forms & Payments

ONLY available through the online store, and the store is ONLY open until Aug. 31.
